The Great Realm of R'Lyeh
Species name: Star-Spawn
Species adjective: Star-Spawn
Empire name: The Great Realm of R'lyeh
Empire Adjective: R'lyehan

Traits: Venerable, slow breeders, repugnant
Ethos: Spiritualist, Fanatic Xenophobe
Government: divine mandate
Homeworld: R'lyeh
Planet type: Ocean World
Short Description of species: Believing themself to have been created by the "Great Old Ones", their gods who they believe ruled the universe but have since left it, leaving them as the (in their eyes) clear heirs to it. They Think that in time they will become their cycle's gods and will force all lesser beings to worship and serve them. They live in the great underwater city of R'lyeh on the planet R'lyeh, a Place where the three-dimensional reality of the universe is not... right. They consider this to be due to the past precense of the Great Old Ones.
Short description of empire: Serving under the the high priest of R'lyeh, who is believed to still be able to communicate with the Great Old Ones The Great Realm of R'lyeh has now discovered their first warp drive, a gift from the Great Old Ones they say and are now ready to conquer the galaxy.
Yes it is a blatant ripoff from H. P. Lovecraft, I also wonder how much of the technology of the Great Old Ones they are able to uncover for the sake of balance?

Military: 5, While their military technology helps and they fill out their ranks with Deep Ones there are relatively few Star-Spawn and their fleet is not very large.
Economy: 3 The extraction of resources is considered unworthy, although they have set sea creatures, and recently Deep Ones, on it and wealth is not a measure used.
Technology: 6, their technology is as immense as it is sporadic, due to their way of gaining it, usually through gifts or relics of the Great Old Ones. As an example they have gained enough psychic powers to have some measure of telepathy and they can use it to more quickly understand languages, on the other hand they have not developed a simple translator to do this for them.
Diplomacy 2: while their psionics can help them they are greatly weakened in the diplomatic field by their immense pride.
Culture: 3, (how do you even put a value on this) An odd culture dedicated to two things, worshipping the Great Old Ones and becoming gods themselves. Their architecture is also very strange due to it being adapted to the fact that R'lyeh can seep into the fourth dimension and they try to turn more planets into R'lyeh in this regard.

(They can probably recover pretty much for the sake of balance. We have one nation advanced in science because of themselves, one because of attacking "the old gods", and one because of gaining it from the "Divine Beings.")

Does not fit, if the setting had allowed it I would probably have made them a fallen empire because that is how I imagine them.
